Days 0–7Goal: Manage swelling, begin restoring motion
Walk with a walker or cane for support and balance
Ice and elevate frequently — swelling typically peaks around day 3

Physical therapy often begins within the first few days after surgery
Motion Goal
By End of Week 1

~90°
Knee Extension (straighten)

Weeks 1–3Goal: Progress motion, reduce reliance on assistive device
Continue formal physical therapy 2–3x per week
Progress from walker to cane as strength and balance improve
Continue daily home exercise program between PT sessions
Gradually increase walking distance and standing tolerance
Weeks 3–6Goal: Restore functional strength and motion
Most patients are walking without an assistive device by the end of this window
Begin strengthening exercises — stationary bike, light resistance work
Driving typically resumes once off narcotic pain medication and reaction time feels normal
Continue working toward full knee flexion and extension
Motion Goal
By Week 6
Knee Flexion (bend)
110–120°
Knee Extension (straighten)

Low-impact activities such as swimming, biking, and golf are commonly reintroduced
Improvement in strength and stamina continues for several months
Note for physical therapists: This protocol is a general guideline. Progression should always be individualized based on intraoperative findings, range of motion achieved, and patient presentation. Please contact our office with any questions about a specific patient's protocol.
